GPCS is the single point of entry in Goochland and Powhatan counties for the Developmental Disabilities (DD) Waiver Wait List. Developmental Services Case Management Services are available to individuals who have a DD Waiver. We actively engage and support children and adults with developmental disabilities to live full lives in the community.
What Case Managers do:
Provide person-centered services.
Assess needs based on history, diagnosis, strengths, and resources.
Assist in making a support plan.
Link to services and supports.
Assist in accessing services and supports.
Coordinate care with other agencies and providers.
Monitor supports and services to ensure individual satisfaction, health, and safety.
Enhance connection and involvement in the community.
Educate about available resources and programs.
Advocate for the individual.
If you would like to access the Developmental Disabilities Waiver Waitlist, you may come in person, without a prior appointment, to be screened at one of the following locations during the designated days and times:
